Assassin's Creed: Embers is a short film, set to debut with the Collector and Animus editions of AC Revelations.
It features Ezio as a very old (70+) man in a new place, most likely Spain/Italy.

This is very spoilerific for Revelations IMO and ruins the whole point of the game. Then again, the story between Rev and AC 2 is basically explained in the book: AC The Secret Crusade.

Just let me know if you want to know what happens in Secret Crusade.

He could suddenly keel over and die from a heart attack due to his condition. Doubt he can be revived, unless they BS some tech through from a vault or something.
